Working with Blackburn CCG and Blackburn with Darwen Borough Council, we delivered a residential intermediate care model providing both ‘step-up’ and ‘step-down’ beds.

The service supports people at risk of unplanned hospital admission and enables patients to leave hospital sooner, with ongoing rehabilitation before returning home. The scheme:

• Reduces pressure on acute and sub-acute services
• Strengthens prevention, discharge and aftercare pathways
• Aligns with the Council’s residential intermediate care strategy
• Cuts admissions and length of stay while supporting safe discharge
